Thank you for your interest in becoming a New Patient at Family Medicine of Michigan. We have implemented a system to streamline the process of becoming a new patient.

The first step is to print off our New Patient Application. The application must be filled out in its entirety and returned to our office. Upon receipt, you will receive communication from us within 2-3 weeks, notifying you of the status. You can also find information as to the insurance companies that we participate with by visiting this page. Please note at this time we cannot see patients with BCN Medicare Advantage or any type of Medicaid plan as we do not participate with these insurances. We are also not accepting uninsured patients.

Please be aware that there is a 4-6 month waiting period to see any of our physicians and a 3-6 week wait to see our nurse practitioners or physician assistant. If your medical needs require more immediate attention, we suggest that you seek care at an emergency department if they are emergent or a ready-care facility for non-emergent services. If this timeline does not meet your needs for things such as prescription refills, we suggest that you continue your search for other primary care offices that may be able to meet your specific needs.

It is very important that our process be adhered to as to allow fairness to all patients seeking to establish care at our office. We are unable to expedite your request as all requests are handled in the order as to which they are received.
